For our non-profit clients, the following questionnaires may be completed on-line. These questionnaires will assist you in gathering much of the non-financial information necessary for us to prepare your 2008 Form 990. Please complete the questionnaire for all organizations as well as the additional one for your specific entity type. If you prefer, you may simply print out the questionnaire and complete it, and return to us via e-mail, fax, or mail.
